I certify that I am the person listed above and a tenant listed on the current lease. All information above is true and accurate to the best of my knowledge. I understand Integrated Property Services may need to contact me before dispatching a repair person to obtain important information. I understand that if I do not respond to a phone call or e-mail with the requested follow-up information a repair person may not be dispatched until such information can be obtained. This is not an urgent repair and life is not in jeopardy and further damage to property will not occur without the prompt response of a service person. Should this be an emergency repair be please call 443-643-0151 immediately. For rental properties managed by Integrated property services, I agree that I have reviewed my lease agreement and I am aware that if I am reporting an item to be repaired, I may be billed a repair deductible in accordance with the terms of my lease. I further agree if the item or items in need of repair were damaged as a result of my own negligence I will pay the full cost of the repair or repairs in accordance with the terms of my lease agreement.
